Registration of New Patients
We welcome requests for registration from patients already living in our practice area, or who are moving into it. You can register by presenting your medical card or completing a form GMS1, which is available from the reception desk. Patients are registered with the practice, rather than an individual doctor and may not always see the same doctor. However, patients can express a preference of practitioner. We will try to comply with your request, but it might not always be possible, in which case we will explain the reasons, but apart from holidays or sickness patients will be able to see a doctor of their choice within four weeks. If you move out of our area, we regret that you must register with another practice.
In addition:
- Registered patients aged 16 - 75 who have not been seen for 3 years may request a consultation
- Registered patients aged over 75 years who have not been seen in the previous 12 months may request a consultation
- If you are unable to attend the surgery for these checks because of your medical condition, a home visit may be arranged.
Pre Registration
Prospective patients can send registration information such as name, address and previous GP details to Ynysangharad Surgery before they attend the practice to register, this can save both patient and Practice time and ensure all relevant information is collected.
